Its the case rim which is getting a slight dent on all of the mis-fires. I suspect it's happening on all the rounds, but the fired rounds iron the dent back out as the bullet leaves. I will definitely look at the local dive shops as I've already murdered the wallet with this pistol. Tell the shop that you want a dive cylinder/valve, and something that will let you screw in a DIN fitting (which is what the end of your adaptor will be.)
