Basically, all of the top shooting clothing makers are based in Europe, with the exception of Marksman House, who serve the Asian market. Kurt Thune, Sauer, Kustermann, Monard, Hi-Tex, Mouche and Anschutz are your options. All make very high quality clothing with differences in construction and materials; but they all can and do make clothing that fits very well, IF you are measured correctly.
There are just a few of us in the states who are dealers and can measure you and place your order. It is also possible to order directly from some of those makers, but not all. Several sell only through their dealer network. If you want more information or want help getting measured I may be able to help you, or point you to someone who can, depending on where you live. please send me an email at info -at- 10pt9 -dot com.

Larry has hit the nail on the head:

The experience and ability of the measurer is a very important element in the process. Ideally whoever measures you will also be able to carry out adjustments to the finished article to achieve an even better fit because no matter how good the measurement and production it's what the garment feels like when worn that matters.

It is also worth considering, that if you are nearish to a "stock size" then a skilled tailor(ess) might be able to produce as good a result from an off the peg jacket as going down the made to measure route.
