Looking for Certificates

I'm looking for a scanned copy of a "President's Hundred or Governors Twenty" type certificate. A digital Photo, or electronic file would do as well. We're trying to start such a program here in Georgia and I need to see what's already out there before I try to "re-invent" the wheel.
