NRA Score reporting forms where to download?

for sanctioned NRA competitions?
I had a link a while ago and I lost it.
This would be the one where you list the shooters and the scores, and mail in the money, all for national ranking.

Actually, they have changed their requirements such that you can send in a computer generated list of your design as along as it includes their minimum info. Note the quote (shown below) at the bottom of their "Tournament Reporting" page at http://www.nrahq.org/compete/dept-tourn-rpt.asp.
Since many sponsors use computers for their match statistics, we will accept a printed score report (sent by "snail mail") provided the following information is included in the report:

NRA ID #
Total number of shots
Total aggregate score (no averages)
Name and address of competitor
