Model 41 safety...... AD?

Practicing out back after work today taking advantage of the mid 60°
temps. Gun is one of the alphabet mod 41's (3 alpha prefix) that
happens to have a slightly better imo trigger than my early A. Been
shooting this for a year now, matches and practice.

I was in process of a sustained portion when shot did not break after
pressure on trigger. WITH FINGER OUT OF TRIGGER, FRONT PAD of trigger
finger ON OUTSIDE OF FRONT OF TRIGGER GUARD, I benched the gun
(maintaining grip) left thumb LIGHTLY brushed slide in preparation to
grip and pull back to see if there was a failure to pickup the next
round, therefore an empty chamber......BAM, round went through my
pistol soft case and out throught the plywood top at about a 45° angle
100% straight away from my body/straight down range. It was fairly
muffled with the very lower edge of the muzzle being in contact with
my soft case. In disbelief, my first thought was that I brushed the
trigger in attempting to manipulate to inspect the chamber.........My
finger was straigt past the trigger and in the position as stated
above. Slide did not contact my hand on recoil/chambering next
round. As I said, must have barely brushed the rear of the slide and
low at that in prep to grasp and ease it back for inspection.

I'm thinking that this could be a true AD due to malfunction ?
However, I'd say the greater question is this 41's safety/
reliability. It was thoroughly cleaned, grips off, tooth brush, mag
disconnector functioning freely, no grit or bind etc. abt 200 rds
ago. Usually clean at 400 rds +/-. Any ideas or suggestions?

I don't care for the idea of aborting a SF shot after some trigger
pressure and possibly having a reoccurrence for whatever reason,
regripping, changing dot intensity etc. or just starting again.

I'm thinking that the slide was barely out of battery and when it had
just enough pressure applied ..................I had a firing pin
released.

This can happen if the over travel stop is set too close. I was shooting a Ruger Mark II when I felt the trigger release to the stop without the gun going off. I squeezed the trigger very hard and managed to get the shot to release, but I should have just pulled back the bolt.

Now I adjust the trigger stops on my guns so that they will fire with 1 piece of printer paper between the stop screw and frame or trigger, but not 2 pieces. This gives me enough extra travel to avoid the above scenario.
